Empower ESTHERs and Caregivers in Managing Medical Documents to Prevent Miscommunication

Health Pocket is a desired communication tool that ESTHERs think that can help them to better manage their medical documents and better communicating with the HCPs. As ESTHERs feel more ‘incharge’ of their folders and their health information within, they could direct intermediate and long-term care providers to refer to it and document the home and community interventions. This can also potentially enable more efficient information sharing.
