Background: Population health can improve much more. The in-flux of new disease into the population can be modelled and predicted. Can it be prevented?

Method: Predicitive models for new cases of 100+ diseases are implemented and tested to try to aid the population on their journey towards health, or put differently, aid the population to not get sick.

Result: The data system displays risks on an individual level for persons currently un-diagnosed, but at risk for disease. This tool is tested on primary care units to delevop a work concept around it. Long term improvement is asessed on incidense rate of new disease.
