Large system improvement strategy from micro-meso-macro-meta system design and cascading measures

The story of a 5+ year national improvement network focused on people with Advanced Cystic Fibrosis Lung Disease exploring Lung Transplant is shared including patient driven origins, how the micro-meso-macro and meta systems provide the framework to design improvements within and between microsystem for people with CF.  Key interventions at all levels of the organization, and an essential core measurement tracking system is discussed with results to date.  This multi-year improvement program design and lessons learned can be adapted for any population or context.
